Supporting Goal-Oriented Primary Health Care for Seniors with Complex Care Needs Using Mobile Technology: Evaluation and Implementation of the Health System Performance Research Network, Bridgepoint Electronic Patient Reported Outcome Tool

We also included monitoring protocols identified as important to these patients including: weight, blood pressure, heart rate, blood glucose, mood and emotion, sleep patterns, diet, and physical activity and walking logs. These protocols had been previously developed and tested with seniors by our technology partner. Patients, their caregivers, and providers can track patient progress on goals and view their symptoms, vitals, and outcome data on the portal.
