The aim of this study was to develop a preventive health screening procedure for home-dwelling older adults aged 75 years to enable the use of clinical patient data for the strategic planning of supportive services in primary care. The Finnish health care and social services system is currently undergoing a major reform [27]. Since the beginning of 2023, these services have been organized by 22 well-being service counties [28,29].

In a previous study [22-24], we developed a preliminary model for resource planning; here, we present an extension of the model with a particular focus on the assessment of the predictions. The model presented here has been assessed comparing the simulated and predicted resource values with the measured ones. The rapid diffusion trend in high-income countries’ populations and in cities with high density stressed the health structure in many countries.

Moreover, we also test the prediction that social cognitive variables targeted by the tailored intervention (ie, self-efficacy and planning) increase more from the baseline in the intervention group than in the control group.
