This study evaluates a peer-supported, transdiagnostic m Health program for trauma-exposed veterans. Given the frequency of comorbid symptomatology among trauma-exposed populations [1,28], a transdiagnostic approach, if successful, provides the opportunity for broad use across various trauma-exposed populations with diverse needs. Moreover, different types of symptoms share issues with emotion regulation and psychosocial functioning, making these difficulties important treatment targets.

This was the first online transdiagnostic prevention program targeting the LGB group based on the ACT framework. Despite previous calls to use standard ACT [40] or affirmative ACT [41] for LGBTQ+ communities, this is the first ACT-inspired open trial tailored for LGBTQ+ people. It is not, however, the first mental health program tailored to the LGBTQ+ community. For example, Pachankis et al [26] used a 10-session LGBTQ+ tailored CBT.

Notably, CBT treatment protocols for anxiety disorders with proven effectiveness span from disorder-specific to transdiagnostic treatment protocols (eg, Dalgleish et al [5]). Despite their evidence base and recommendations by current treatment guidelines (eg, National Institute for Heath and Care Excellence treatment guidelines), there remains significant room for improvement. Only about half of the individuals treated with CBT experience full remission or clinically meaningful symptom reduction [6,7].

Initially, the PSPNET team adapted an existing transdiagnostic ICBT course that had demonstrated effectiveness in the general population in Australia and the Canadian province of Saskatchewan [30-33]. This course was tailored for Canadian PSP and was titled the PSP Wellbeing Course. Initial research results from the PSP Wellbeing Course showed that tailored ICBT was used by PSP to address both occupational and personal concerns [34] and was viewed as beneficial [35].
